Mid Year Report Form (Google Doc On-line Form)

Dear Seniors:
As you are probably (and hopefully) aware, ALL colleges require your Mid-Year Transcripts as part of your application. If the colleges do not receive your Mid-Year transcripts, they will consider your application as "incomplete," which will cause them to automatically reject it.
I will only send the mid-year transcripts for those students who complete this form. Please complete this form by Wednesday, February 3rd by 11:59:59 pm.

FYI: Many colleges have requested to receive your Mid-Year grades by February 1st. However, your colleges are aware that the official last day of the Fall 2009 semester is February 1st for all New York State public schools (and many private schools.) Therefore, your colleges will accept them after this date. If this presents any particular difficulties, please contact me or the Guidance Department Assistant, Ms. Ortiz (iortiz@hsmse.org).
